The Penn Center for Living Donation cares for patients throughout the referral, evaluation, pre-donation and post-donation phases of care for Kidney, Liver, or Uterus Donation. The living donor coordinator is responsible for referrals, screening potential donors, assessing for compatibility, providing education, facilitating consents, managing patient evaluations, coordinating donations, and maintaining accurate donor records. Our Center for Living Donation Team is committed to advancing our vision of collaborative donor-focused patient management. The Transplant Coordinator II is a professional nurse who safely practices with the UPHS professional practice model and demonstrates UPHS core values that reflect its mission. The Transplant Coordinator II provides direct care and coordinates care for patients in all phases of the donation process. The patients range in age from young adult to elderly. The Transplant Coordinator II demonstrates competency in patient and customer care, age-specific accountabilities, patient/employee safety accountabilities, operations, and core values. The Transplant Coordinator II assesses and evaluates patients and families developing a care plan in collaboration with the multidisciplinary Living donor team. The Transplant Coordinator II creates a written, continuous record for each patient; implements identified medical and nursing interventions; evaluates outcomes. The Transplant Coordinator II serves as an integral member of the transplant team in meeting the needs of the donor and ensuring continuity of care throughout the donation process. The Transplant Coordinator II maintains competencies for transplant patients' care and demonstrates the skills and knowledge to provide ongoing, individualized care.
