Description: This nurse will work in the designated territory seeing patients in the field on hospice services. This is a weekend program from 8 AM to 8 PM every Saturday and Sunday. This is not a case management position and does not carry a case load. Visit types include daily end-of-life visits, PRN visits for symptom management, and occasionally stable routine visits. Staff will primarily be seeing patients in community settings, such as their homes, assisted living facilities, group homes, nursing homes, and perhaps hospice patients in hospitals. Hospice patients are at end of life with a life expectancy of 6 months or less if the disease runs its normal course. Common diagnoses consist of 40% cancer and 60% other medical issues. The remote/field RN caseload is approximately 13-15 visits per day prorated for a full shift. Required skills include IV skills, chest tube maintenance, catheter insertion, tube feedings, and wound care. Comfort with end-of-life care, including pain and symptom management, is essential. Independent critical thinking and problem-solving skills are necessary. Flexibility is important as the inpatient facility provides palliative sedation when needed. Technology used includes EMR systems and various IV pumps. Family, patient, and caregiver typically administer medications, while IVs/injections are managed by the RN.
